​​​Summary of our Terms 

​

These Terms of Service (“Terms”) are part of the Liability Release Consent Agreement – a legally binding contract governing your participation with Canine Assistance Puerto Rico. You should read these Terms of Service (“Terms”) in full, but here are a few key things you should take away:

 

  • We appreciate your understanding of our policies, which are designed to provide immediate access to online platforms.

 

  • No refunds are given after training begins.

 

  • Cancellation/no refund/no show policy:  We can no longer offer refunds for cancellations {we still get charged by the card processing company, even when you are refunded the whole amount} cancellation fee $650 due immediately.  However we are more than happy to accommodate your needs by rescheduling your appointment  to a different session or term when at least 24 hour written notice is given.

  • We will always answer an email inquiry within 48 hours.  If you have not received an email from us, please check your junk or promotions folder.

​

Please also note that these Terms incorporate our Privacy Policy, (https://www.canineassistancepr.com/privacy) and  Service Dog Eligibility Policy (https://www.canineassistancepr.com/sdeligibility) as well as other terms applicable to your use of the Services and The Course Plan and Learning Objectives, completing Liability Release Consent Agreement, NonDisclosure Agreement, Media Release, and Service Animal Training and Support Acknowledgement.  Finally, you must be at least 18 years old to Consent to services and at least 13 years old to participate in training.

Terms of Service​

**Weather related changes:  In the event we need to cancel due to hurricanes or unforeseen circumstances, class will not be held, no notice may be received due to island wide power outages. Classes will resume as previously scheduled once power is restored.  Projected end dates may be affected to accommodate missed sessions.

 

Financial Agreement:

Clients are required to pay class fees as outlined in this Financial Agreement. All fees are non-refundable once training begins.  Please check your availability before purchase.  Payment must be received first and then scheduling of each individual appointment is made to the appointment calendar.  

 

Due to limitations on class sizes if you do not book your individual sessions, you may be subject to not being able to fit into the schedule for a period of time.

Code of Conduct:

Participants agree to:

​

  • Treat their dog with respect and kindness.

  • Abide by all training protocols and best practices.

  • Be on time for classes and being on time means arriving 5 minutes or more to the waiting rom to ensure prompt start and finish.

  • Communicate promptly within 24 hour written notice about any missed session.

  • Maintain respectful behavior towards all instructors, staff, volunteers, and/or other participants.

  • Students in re-certification and follow-up evaluations as required to maintain Certified Service Dog status.

Additional Client Requirements:

  • Clients are encouraged to maintain a regular therapy or counseling regimen with a registered mental health provider.

  • Clients must maintain a clean and sober environment during training sessions.

  • Dogs must be clean and well groomed in public.

​

We will address routine training challenges during regular class times, but any immediate concerns will be evaluated by instructors for appropriate action.

​

Re-certification and Follow-Up

Clients will need to complete a timely re-certification to ensure their dog maintains Certified Service Dog status. This includes passing the Public Access Test, meeting all course benchmarks, and submitting an up-to-date Veterinary Recommendation Form.

​

By enrolling in the Canine Assistance Puerto Rico Owner Training Programs or classes, clients agree to adhere to all policies and uphold the standards and expectations set forth for both their dog and themselves. This includes compliance with all health, behavioral, and training requirements, as well as the Canine Assistance Puerto Rico Code Of Conduct and the gold standard of behavior for dogs in our communities.

 

If at any time during or after the course, a participant or their dog fails to meet these standards, Canine Assistance Puerto Rico reserves the right to discontinue training, remove the dog from the program, or take any necessary action to protect the well-being of the dog or the public.

Consumer Disclosure Regarding Conducting Business Electronically, Receiving Electronic Notices and Disclosures, and Signing Documents Electronically

Please read the following information, by proceeding forward and signing this document you are agreeing that you have reviewed the following consumer disclosure information and consent to transact business using electronic communications, to receive notices and disclosures electronically, and to utilize electronic signatures in lieu of using paper documents. This electronic signature service is provided on behalf of clients, “sending party,” whom are sending electronic documents, notices, disclosures or requesting electronic signatures to you.

You are not required to receive notices and disclosures or sign documents electronically. If you prefer not to do so, you may request to receive paper copies and withdraw your consent at any time as described below.

​

Paper Copies

You are not required to receive notices or disclosures or sign documents electronically and may request paper copies of documents or disclosures if you prefer to do so. You also have the ability to download and print any open or signed documents sent to you through the electronic signature service using the PDF and Print icons. Adobe may also email you a PDF copy of all agreements you sign using the electronic signature service. If you wish to receive paper copies in lieu of electronic documents you may close this web browser and request paper copies from the “sending party” by following the procedures outlined below. The “sending party” may apply a charge for requesting paper copies.

Use of the Service requires a standards-compliant web-browser which supports the HTTPS protocol, HTML, and cookies. Viewing PDF documents requiring additional software such as Adobe Reader, Adobe Scanner, or similar.

 

Withdrawal of Consent

You may withdraw your consent to receive electronic documents, notices or disclosures at any time. In order to withdraw consent you must notify the “sending party” that you wish to withdraw consent and to provide your future documents, notices, and disclosures in paper format. After withdrawing consent if at any point in the future you proceed forward and utilize the electronic signature system you are once again consenting to receive notices, disclosures, or documents electronically. You may withdraw consent to receive electronic notices and disclosures and optionally electronic signatures by following the procedures described below.

Scope of Consent

You agree to receive electronic notices, disclosures, and electronic signature documents with all related and identified documents and disclosures provided over the course of your relationship with the “sending party.” You may at any point withdraw your consent by following the procedures described below.

 

Requesting paper copies, withdrawing consent, and updating contact information

You have the ability to download and print any documents we share in Teams. To request paper copies of documents, withdraw consent to conduct business electronically and receive documents, notices, or disclosures electronically or sign documents electronically please contact the “sending party” by telephone, postal mail, or by sending an email to the “sending party” with the following subjects:

“Requesting Paper Copies” Please provide your name, email, telephone number, postal address and document title.

“Withdraw Consent” Please provide your name, email, date, telephone number, postal address.

“Update Contact Information” Please provide your name, email, telephone number and postal address.

Any fees associated with sending paper copies or withdrawing consent will be determined by the “sending party.”

 PRIVACY

This notice describes how your personal information about you may be used and disclosed and how you can get access to it. Please review carefully.

​

Your personal information including medical or health records may be used to provide training services, and billing. Examples of these activities include but not limited to review of records is to ensure appropriate training, electronic, or mail delivery of billing for services to you or other authorized payers, appointment reminder telephone calls, and records review to ensure completeness and quality assurance. Use and disclosure of your records is limited to the internal used outlined above except required by law or authorized by the client or legal representative.

Disclosed personal information will be limited to the minimum necessary. You may request an account for any uses or disclosures other than those described above.  Access is also available under a logged in profile.

​

You, or your legal representative, may request your records to be disclosed to yourself or any other entity. Your request must be made in writing, clearly identify the person authorized to request the release, specify the information you want disclosed, the name and address of the entity you want the information released to, purpose and the expiration date of the authorization. Any authorization provided may be revoked in writing at anytime. Service dog application (electronic form and contract) are part of your personal records. We have 30 days to respond to a disclosure request if available.

You may request corrections to your records.

A request for disclosure may be denied under the following circumstances: disclosure would likely endanger the life or physical safety of you or another person, requested information references other persons, or if released to a legal representative would likely result in harm.

If a request for disclosure is denied for reasons outlined you or your legal representative may request review of the denial. A review will be concluded within 30 days.

​

You may request that we restrict uses and disclosures outlined above. However, we are not required to agree to the restrictions. If an agreement is made to restrict use or disclosure, we will be bound by such restriction until revoked by you or your legal representative orally or in writing except when disclosure is required by law or in an emergency. We may also revoke such restrictions but information gathered while required by law or in an emergency. We may also revoke such restrictions but information gathered while the restriction was in place will remain restricted by such an agreement.

If you wish to complain about privacy related issues you may contact CANINE ASSISTANCE PUERTO RICO, LLC. 1654 CALLE TULIPAN, SUITE 100, San Juan, PR 00927. In any case there will not be any retaliation against you or your legal representative for filing a complaint.

Si desea recibir copias en papel en lugar de documentos electrónicos, puede cerrar este navegador web y solicitar a la "parte transmisora" que le envíe copias en papel siguiendo los pasos descritos abajo. La "parte transmisora" podría aplicar cargos por el envío de copias en papel.

Revocación del consentimiento

En cualquier momento podrá revocar el consentimiento para dejar de recibir revelaciones, documentos o avisos electrónicos. Para revocarlo, debe comunicar a la "parte transmisora" su deseo de retirar el consentimiento y de recibir, a partir de ese momento, las revelaciones, los documentos y los avisos de forma impresa. Si, tras revocar el consentimiento, decide utilizar el servicio de firma electrónica en algún momento, estará volviendo a dar su consentimiento para recibir avisos, revelaciones o documentos en formato electrónico. Puede revocar su consentimiento para dejar de recibir revelaciones y avisos electrónicos, así como firmas electrónicas si lo desea, siguiendo el procedimiento descrito abajo.

 

Alcance del consentimiento

Durante la relación con la "parte transmisora", acepta recibir revelaciones y avisos electrónicos, así como documentos con firma electrónica y toda la documentación y las revelaciones asociadas e identificadas que se suministren. En cualquier momento podrá revocar su consentimiento siguiendo el procedimiento descrito abajo.

Solicitud de copias en papel, revocación del consentimiento y actualización de los datos de contacto

Si lo desea, puede descargar e imprimir los documentos que le enviemos a través del sistema de firma electrónica. Para solicitar copias en papel de los documentos, revocar su consentimiento para realizar actividades comerciales electrónicamente y recibir revelaciones, documentos o avisos electrónicos, así como firmar documentos con firma electrónica, póngase en contacto con la "parte transmisora" por teléfono, correo postal o correo electrónico con los asuntos siguientes:

"Solicitar copias en papel": indique su nombre, correo electrónico, número de teléfono, dirección postal y el título del documento.

"Revocar el con consentimiento": indique su nombre, correo electrónico, fecha, número de teléfono y dirección postal.

"Actualizar los datos de contacto"; indique su nombre, correo electrónico, número de teléfono y dirección postal.

La "parte transmisora" fijará las tarifas correspondientes al envío de las copias en papel o a la revocación del consentimiento.

 

Intimidad

Este aviso describe cómo se puede utilizar y divulgar su información personal sobre usted y cómo puede acceder a ella. Por favor, revíselo cuidadosamente.

Su información personal, incluidos los registros médicos o de salud, se puede utilizar para proporcionar servicios de capacitación, facturación y recibir pagos. Ejemplos de estas actividades incluyen, entre otros, la revisión de los registros es garantizar la capacitación adecuada, la entrega electrónica o por correo de la facturación de los servicios a usted u otros pagadores autorizados, las llamadas telefónicas de recordatorio de citas y la revisión de registros para garantizar la integridad y la garantía de calidad. Usted, o su representante legal, pueden solicitar que sus registros se revelen a usted o a cualquier otra entidad. Su solicitud debe hacerse por escrito, identificar claramente a la persona autorizada para solicitar la divulgación, especificar la información que desea revelar, el nombre y la dirección de la entidad a la que desea que se publique la información, el propósito y la fecha de vencimiento de la autorización. Cualquier autorización proporcionada puede ser revocada por escrito en cualquier momento. La solicitud de perro de servicio (formulario electrónico y contrato) forma parte de sus registros personales. Tenemos 30 días para responder a una solicitud de divulgación si está disponible.

Una solicitud de divulgación puede ser denegada en las siguientes circunstancias: la divulgación probablemente pondría en peligro la vida o la seguridad física de usted u otra persona, la información solicitada hace referencia a otras personas, o si se entrega a un representante legal probablemente resultaría en un daño.

 

Si se deniega una solicitud de divulgación por las razones descritas, usted o su representante legal pueden solicitar la revisión de la denegación. Una revisión se concluirá en un plazo de 30 días.

Puede solicitar que restrinjamos los usos y divulgaciones descritos anteriormente. Sin embargo, no estamos obligados a aceptar las restricciones. Si se llega a un acuerdo para restringir el uso o la divulgación, estaremos obligados por dicha restricción hasta que usted o su representante legal lo revoquen oralmente o por escrito, excepto cuando la divulgación sea requerida por la ley o en caso de emergencia. También podemos revocar dichas restricciones, pero la información se recopila mientras así lo exige la ley o en caso de emergencia.
